The team at the Auckland City Hospital that had attended to the woman had replaced the first Alexis wound retractor they used with a larger one. It was this second retractor that was left in the woman's abdomen.
The Commissioner noted it was the second time in two years that a device had been left in an Auckland hospitals' patient.

Researchers warn the case highlights the increased danger of diseases and infections being passed from animals to people.
The Ophidascaris robertsi roundworm is common in carpet pythons - non-venomous snakes found across much of Australia.
Scientists say the woman most likely caught the roundworm after collecting a type of native grass, Warrigal greens, beside a lake near where she lived. The area is also inhabited by carpet pythons.
Writing in the journal, Australian parasitology expert Mehrab Hossain said she suspected the woman became an "accidental host" after using the foraged plants - contaminated by python faeces and parasite eggs - for cooking.
"The invasion of the brain by Ophidascaris larvae had not been reported previously," writes Dr Hossain.
"The growth of the third-stage larva in the human host is notable, given that previous experimental studies have not demonstrated larval development in domesticated animals, such as sheep, dogs, and cats."
Dr Senanayake - who is also an associate professor of medicine at the Australian National University (ANU) - told the BBC the case is a warning.
The ANU team reports that 30 new types of infections have appeared in the last 30 years. Three-quarters are zoonotic - infectious diseases that have jumped from animals to humans.
"It just shows as a human population burgeons, we move closer and encroach on animal habitats. This is an issue we see again and again, whether it's Nipah virus that's gone from wild bats to domestic pigs and then into people, whether it's a coronavirus like Sars or Mers that has jumped from bats into possibly a secondary animal and then into humans."

NZ still gets a few nasty spiders (Red Back, Katipo, White Tail) , White Nurse Sharks, Bluebottles, some toxic sea slugs as well as bad tempered Sea Lions and Wild Boars.  The possums also have a bad attitude but are not particularly deadly.

Missing F-35: US military asks for public's help to find jet

The US military has asked for the public's help to locate one of its $80m (£65m) F-35B fighter jets after the pilot ejected from the aircraft.
It went missing on Sunday afternoon when the pilot was flying over the southern state of South Carolina.
The pilot, who has not been named, ejected and parachuted safely. He is in a stable condition in hospital.
It is not yet clear what happened, but officials said the aircraft was involved in a "mishap".

As the ex-US-Military jet pilots in the stream linked above have commented, the wingman in the other Marine F35 would have followed procedures and kept an eye on the ejected pilot to identify where they landed, by which stage Amy seems to have just waddled its fat ass off across the countryside by itself on autopilot (prob just on yaw damping and roll/pitch hold, rather than any particular course and altitude).  With luck it eventually headed out to sea and came down in the ocean rather than landing on someone's garden shed or outdoor privy.

There are apparently some conspiracy theories about due to the fact it was a Sunday, but the ex-Marine pilot said the Marines do occasionally conduct some training or test exercises on a Sunday.
